I think the engine manufacturer is catching hell for a bad cowling design and/or maintenance program. Even if the fan blade was the cause, it shouldn't have been able to shake off the cowl.

The CFM56 is one of the safest and most reliable engines built. The blade was pushed forward for some reason into the cowling forward of the fan, where it's no protected from impacts.
